After contacting us to inquire about services by phone (218-576-5757 or 844-576-5757) or email (click here), we will be in touch with you to discuss your needs. The Guidance Group will ask you for the name, date of birth, address, and phone contact information of the primary client along with a brief description of the reason for seeking services and the type of services being requested at this time.
We will discuss payment options with you. Additionally, we will ask for information regarding any insurance coverage to be used for payment, including the health plan name, group and ID numbers, and the billing address and phone numbers for the plan. If the client is a dependent of the primary insured person, we will need information regarding the primary policy holder for billing purposes.
After an appointment is scheduled, we will create a patient portal account for you. You will receive an email with a link to set up the account, and the appropriate clinical intake forms will be available to you in the portal. We request that these be completed in advance. Some can be filled out online, and some can be printed and signed. If you are unable to access a printer, a copy of the forms can be made available to you at your first session (or in the case of Telehealth, mailed to you ahead of time).
